Description of Activities
Please provide description of the activities and services offered by your program including new programming and supports being implemented this year. This can include academic, student enrichment and parent/guardian activities as well as field trips or outstanding one time actvities, etc.
The Bleckley County Primary School after school program ran from 3:00 PM to 6:00 PM, Monday thru Thursday. The program began with a nutritious snack, and enrichment and recreation activities, followed by homework help and tutoring in math, language arts, reading, social studies, and science. Parents were invited to join events, including parent orientation/OFTC presentation and Lights On Afterschool.

The Bleckley County Royal Academy after school program ran from 3:15 PM to 6:15 PM, Monday thru Thursday. The program began with a nutritious snack, followed by homework help and tutoring in math, language arts, reading, social studies, and science. Students then participated in enrichment and recreation activities. Parents were invited to join events, including parent orientation/OFTC presentation, parent literacy activity “Make It and Take It”, and Lights On Afterschool.

We are monitoring the students’ progress towards successfully obtaining this objective through ongoing communication with the students’ regular classroom teacher.

Goal # 3.1:  Each year of the grant, a minimum of 80% of the parents/guardians who attend at least one family related session will state that they have gained increased knowledge regarding encouraging and supporting their child’s academic success. P2. Parent Engagement – Performance 2. Did not meet but progressed toward the stated objective Surveys are distributed at the conclusion of each parent session. An analysis of the surveys is included in our quarterly and annual reports. 61 surveys were received during the first semester.

Parent Engagement – Participation 2. Did not meet but progressed toward the stated objective Parents were invited to attend three events during the first semester. 47 family members of participating students attended the activities offered.
Program Recommendations
Directions: Please provide program recommendations in Column A based upon the results of this Formative Assessment. These recommendations can come from within the program, the external evaluator, or other program stakeholders. Example: The program has made progress but has not met the academic objectives for math and English Language Arts. It is recommended to review student data to identify students who are not showing improvement as well as determine possible interventions and assistance. Individual targeted assistance and tutoring sessions might be helpful based on information provided by regular-school day teachers. It is recommended to communicate consistently with the regular school day teachers to coordinate support and share progress. Example: Leadership Teams at each site will review student data and grades. Based upon data analysis, the program will use the following interventions to revise the program and activities. 1) The program will develop a portfolio for each student that includes student goals and progress. 2) The program will use project based learning to facilitate connections between learning and the real-world. 3) The program will provide families with appropriate activities to use with their child at home. 4) The program will schedule time for conferences with the 21st CCLC staff and regular school day teachers.
Disciplinary referral reports should be submitted to the evaluator quarterly. The program should work with the school to obtain the number of student disciplinary referrals by student for each grading period.  This information should subsequently be forwarded to the evaluator with the quarterly worksheets.
